Please be informed of the following.

1. You cannot cancel or change same-day appointments. There will be a $50 charge unless  you notify us at least 24 hours prior to the scheduled appointment time  (must be during our office hours of 9:30am to 5:00pm).
2. You must notify us to cancel or change a weekend and /or Monday appointment no later then 5:00pm on the Friday before the appointment (s). Otherwise, there will be a $50 charge.
3. Please give us at least 5 days of advanced notice when scheduling  appointments online.
4. We will confirm your appointment via e-mail or phone. Otherwise, the appointment will not be set.
